    Athletic physicals may be received at the Family Practice in Atoka for the school cost of $10. Please stop by the office or print the physical form prior to the evaluation.

Health Note!

  Effective for students ages 12 & younger: The Advisory Committee on   ImmunizationPractices (A.C.I.P) recommends a second dose of varicella
 (chicken pox) be administered. This second vaccination is not required for school entry, but is highly recommended!
